Nonsurgical Weight Loss Care

Those options include a procedure without the need for an endoscopic (down your throat) and another one especially designed for patients with Type 2 diabetes.

Below are some of the nonsurgical services we offer:

Elipse gastric balloon

This nonsurgical gastric balloon weight loss treatment also requires no anaesthesia or endoscopy — and takes just 15 minutes to complete. You can expect to lose from 10-20 kg in four months through this safe and effective treatment. We’ll follow-up with a 6-month weight loss programme that includes a healthy diet and regular exercise.

First, you’ll swallow a small capsule containing the Elipse balloon, which is attached to a tube. You’ll then get an X-ray to be sure the balloon is in the correct position. We then fill the balloon through the tube with purified water and then remove the tube.

A full balloon will give you the sensation of fullness in the stomach, which makes it easier to adjust to smaller portion sizes as part of a weight loss programme. And there is no removal process – you’ll naturally pass the balloon through your body four months after insertion.

The balloon helps to set up longer-term weight loss success, supporting you in the earliest stage so that you can see and be encouraged by positive results. We’ll also help you learn healthy eating habits to enable you to continue your progress after the balloon has gone.

Orbera gastric balloon

If you have a Body Mass Index (BMI) over 27 and have not been able to achieve weight loss through diet or other methods, the Orbera365™ gastric balloon could be an effective solution. You can expect to lose about 20kg over six months with this procedure.

You could also be considered for this balloon if you have a BMI of 35 to 40, but don’t want to undergo surgery — or if you need to lose some weight before undergoing weight loss surgery. You might be also a candidate for this gastric balloon if you’re being prepared for orthopaedic, cardiac or other surgery and need to lose weight in advance.

In this procedure, your doctor will insert a gastric balloon device through your mouth and into your stomach. We’ll then fill the balloon with saline fluid, reducing stomach capacity. This will produce a feeling of fullness after eating small portions of food, helping you learn to eat less and to choose better foods.

You’ll be mildly sedated and the procedure should take about 30 minutes. You’ll recover quickly and you won’t have any incisions or scars — and no overnight hospital stay.

The balloon will remain in your stomach for about six months, when your doctor will remove it. Your consultant will then talk with you about any further treatment.

Spatz3 adjustable balloon 

The Spatz3 gastric balloon is a good solution if you have a BMI over 27 and you haven’t been able to achieve weight loss through diet or other methods.

This balloon can be expanded to boost weight loss when your body gets used to it — or actually reduced in size to make it easier to tolerate. You can also use this saline-filled balloon for up to one year, longer than other types of gastric balloons.

You can expect an average weight loss of 16kg after six months — or 25kg after 12 months when used in conjunction with diet, exercise and a change in lifestyle.

You’ll be mildly sedated as your consultant inserts the Spatz3 balloon through your mouth and into your stomach. Again, you won’t have to stay at hospital. This balloon may take up to a third of your stomach volume, helping you to eat less and feel fuller longer.

The balloon will remain in your stomach for about six months, when your doctor will remove it. Your consultant will then talk with you about any further treatment.

You could also be considered for this if you have a BMI of 35 to 40, but don’t want to undergo surgery or need an initial weight loss step before undergoing weight loss surgery. And you might be also a candidate for this gastric balloon if you’re being prepared for orthopaedic, cardiac or other surgery and need to lose weight in advance.

Endoscopic sleeve gastrectomy

We don’t use a balloon for an endoscopic sleeve gastrectomy (ESG) procedure — but it’s also less invasive than surgery. The National Institute for Health and Care Excellence (NICE) approved this procedure in 2024 in the UK for people with a BMI between 35 to 40.

In this procedure, your surgeon inserts a thin, flexible tube with a camera attached to the end, called an endoscope — and then a small suturing device to stitch your stomach to make it smaller. This will reduce the amount of food the stomach can hold.

You won’t have any incisions after this innovative, minimally invasive procedure, which has fewer complications than traditional surgery. You’ll also be mildly sedated and won’t have to stay in hospital overnight.

You could have significant weight loss with this procedure – up to 60% of excess weight – but it’s also reversible. Your stitches can simply be removed, so the procedure can be undone if you’re not happy with the results. ESG can lead to other health benefits, from lowering blood pressure and cholesterol to improving arthritis, sleep apnoea and energy levels.

This is a new procedure and Cleveland Clinic London’s consultants are leading the way in its development. So we can advise on the benefits, risks and potential complications.

Endoscopic sleeve gastroplasty 

Similar to endoscopic sleeve gastrectomy, this procedure is nonsurgical and performed by your consultant with an endoscopic device inserted through your mouth into your stomach — but in this case, your surgeon will alter your stomach from a pouch shape to a sleeve shape. This also reduces the size of the stomach, helping you feel full and satisfied after consuming fewer calories.

This procedure is appropriate for patients with a BMI over 30 and is increasingly popular as it is minimally invasive, with no external cuts or scars. It is also reversible, recovery is quicker than from surgery and most patients do not require an overnight hospital stay.

Duodenal mucosal resurfacing (DMR)

If you have Type 2 Diabetes, this new endoscopic treatment might be for you. The DMR procedure targets the duodenum (part of the small intestine), which often isn’t working right if you have Type 2 Diabetes. Cleveland Clinic London is the only hospital in the world offering this treatment both clinically and in various research studies.

Your consultant will insert an endoscope along with a thin flexible tube filled with hot water to heat and remove the thickened duodenum lining. Your body will then build its own healthy lining, called a mucosal layer. This will then help to control diabetes by controlling blood glucose and insulin resistance more effectively.

You will be under general anaesthesia during this procedure but will likely leave hospital the same day. Following the procedure, you’ll follow a careful diet plan enables the duodenum to heal, after which you can start on a normal healthy diet.
