Hip joint preservation

Joint preservation may be right for you if you have hip damage but want to prevent or delay joint replacement. It can increase mobility and function, prevent further cartilage loss, reduce joint pain, and repair or reverse existing joint damage. We may use it for cartilage injuries, some congenital joint disorders or early-stage arthritis if there is not too much cartilage loss.

Your joint preservation plan may combine conservative (nonsurgical) therapies and surgical procedures.

Nonsurgical therapies may include anti-inflammatory medications, steroid injections, platelet-rich plasma (PRP) therapy and injections of hyaluronic acid (viscosupplementation).

Your consultant may also recommend unloading braces or physiotherapy and weight loss through supervised exercise.

Surgical therapies may include:

  • ArthroscopyHip arthroscopy is a minimally invasive surgery that may help you recover faster and with less pain. We make several tiny incisions and use a small camera and surgical tools to remove torn or damaged parts from your joint. This procedure preserves bone and causes minimal soft tissue damage.
  • Cartilage repair: We use innovative techniques to repair cartilage around joint surfaces. Some treatments involve growing cells to regenerate the joint surface.
  • Osteotomy: In this procedure, we cut and realign bones in your hips to distribute weight more evenly across your joint. If you have hip dysplasia, you may need hip osteotomy — specifically, periacetabular osteotomy.

Hip replacement

If joint preservation is not effective, it may be time for joint replacement. We typically reserve this treatment for people over 60 whose symptoms affect their ability to sleep and do their usual activities. We can often perform hip replacement using minimally invasive resurfacing surgery. Depending on your condition, your consultant may recommend:

  • Total hip replacement: We remove the top part of your thighbone and the damaged cartilage in your hip socket. Then, we replace them with metal implants (stem, ball and socket) and a plastic spacer so the artificial joint glides smoothly.
  • Hip resurfacing: We replace the damaged cartilage lining your hip socket with a metal shell, but leave the ball of your thighbone in place and cover it with a metal cap.
  • Hip revision: We may do hip revision surgery if the original implants are worn or loose, if you have a fracture around the implants or if the original artificial ball-and-socket dislocates.
